In fact, however, such comparisons are rather complicated, because even within one beverage category (e.g., beer, wine, or distilled spirits), the alcohol contents may differ considerably. Many of these trials have been conducted for weeks, and in a few cases months and even up to 2 years, to look at changes in the blood, but a long-term trial to test experimentally the effects of alcohol on cardiovascular disease has not been done. A recent successful effort in the U.S. to launch an international study was funded by the National Institutes of Health. Although the proposal was peer-reviewed and initial participants had been randomized to drink in moderation or to abstain, post hoc the NIH decided to stop the trial due to internal policy concerns. 18 An earlier study suggested that getting 600 micrograms a day of folate could counteract the effect of moderate alcohol consumption on breast cancer risk.
Effects of long-term alcohol use
There is also some evidence that genes influence how alcohol affects the cardiovascular system. One variant of this enzyme, called alcohol dehydrogenase type 1C (ADH1C), comes in two “flavors.” One quickly breaks down alcohol, the other does it more slowly. Thus, we need to rely on short-term intervention studies and epidemiologic studies, each of which has clear limitations in showing an effect of alcohol intake on the vulnerability to gain weight. However, the preponderance of the evidence taken as a whole suggests that alcohol may be a risk factor for obesity in some individuals, especially when consumed in large quantities. In fact, in nutritional epidemiology studies that investigated the consumption of various food categories, reported alcohol intake was particularly reproducible compared with the reported intake of other nutrients (Longnecker et al. 1993).
